They supply a set of checks to ensure the stupid but critical stuff is not overlooked, and they supply another set of checks to ensure people talk and coordinate and accept responsibility while nonetheless being left the power to manage the nuances and unpredictabilities the best they know how.

Rank, titles, or inferred status can impede open communication in an environment where people thrive on helping one another.

Instead of a culture of fear, we have a high-trust, collaborative culture, where people are rewarded for taking risks. They are able to fearlessly talk about problems as opposed to hiding them or putting them on the backburner--after all, we must see problems in order to solve them.
